The topic of palliative care will always be difficult.

End of life choices are a deeply personal topic and given the choice to plan and discuss, everyone has their own opinion on how they would like things taken care of when it comes to their own time.

Surprisingly, you may discover that very few have actually made plans for their end of life.

In a recent survey by Palliative Care Australia, it found just 5 percent of the 1,000 people surveyed had made an advance care plan, setting out how they would like to be cared for at the end of their life.

We're here to support you through this journey.

A care plan helps friends and loved ones make the difficult decisions at the end of your life, and carry out your healthcare wishes in the manner of your choosing.

"These people are outstanding. very generous with their time and patience towards my dementing and often crotchety chair-bound 91yo mother. As a medical practitioner myself I've seen the inside of a lot of nursing homes but SummitCare Waverley really is family-home-like in the attention I've seen the residents enjoy. They deserve every kindness in return."
